Payment for Services

We encourage our patients to become familiar with their insurance benefits. Read your insurance handbook or call your insurance carrier and ask questions. On the back or front of your insurance card you will find a customer service phone number.

If your insurance carrier requires you to make a co-payment or your deductible has not been met, you will be expected to pay those fees at the time of service.

Self-paying patients are required to pay at the time of service and some procedures may require payment before services are rendered. This information will be made available to you when you schedule your appointment and some procedures may require payment from insured patients for services performed.

If you need to discuss your insurance benefit responsibility, please ask to speak to our Business Manager.

You will receive monthly statements from our billing service. If your insurance fails to pay within 45 days please call the customer service phone number to investigate the reason for delay.
